Please download the Information/Registration Form, fill it out, attach your payment in the form of a check made out to "Central Carolina Hockey" (please don't send cash!) and mail it to:

Please include your player's name on the check. You may also FAX it to 919-481-0503, and we will follow up you for payment information. Registrations are processed on a first-come, first-serve basis, and are not final until payment has been provided.
